Eyban PCA Provider LLC is a home health care provider located in Ramsey, Minnesota. We established the company in 2018 and have continued to render our services to our clients up to this day. We serve people of all ages who need quality health care while remaining at home.
Our company and staff shall operate and provide services in compliance with all applicable federal, state, and local laws and regulations, and disclosure and ownership information. To the best of our knowledge, based on our Quality Improvement Program and professional personnel practices, our services comply with professional standards and principles.

Our Mission
Eyban PCA Provider LLC believes that health care is a basic human right. It must be available, coordinated, and provided in a comprehensive way, combined with other human services when appropriate. We believe home health care is an important part of the continuous health care system, and it shall be provided in the most cost-effective way possible. We believe the services should be tailored to meet the needs and wishes of the clients we serve.

Our Responsibilities
We at Eyban PCA Provider LLC are committed to our responsibility in serving our clients and the community. That is why we promote honest and appropriate interactions with clients, and as much as possible, we include them in decisions about their care, treatment, and issues.

Agency responsibilities
a. When a client is admitted for care, the agency has the responsibility of providing services that are appropriate to the needs of the client and are within the mission, laws and regulation, and the agency’s capability. Should there be any conflict coming to the surface that might result in denial of care, the specific needs of the client will determine the decision regarding their transfer or discharge.

Inter-Agency Relationships
a. To provide relevant and timely information, all referral sources shall honestly and conscientiously cooperate to assure appropriate and comprehensive services.

Fiscal Responsibilities
a. In line with the client’s needs, the services they receive will go in parallel with their payment.
About the Founder

- “My name is Abdisa Nagayo. I am from Oromia which is located in East Africa inside Ethiopia. I came to the US at a younger age and I always liked helping people with their needs. I decided to become a nurse to help others in need. For me, nursing was not only a job but it was also my passion since I was a child. When I was a child in Ethiopia, I saw people dying from little diseases such as appendicitis that required minor surgeries. Since then, I have always wanted to make a difference by learning the required skills needed to become a nurse and help those in need. I have worked as a nurse for so many years and I have gained a lot of knowledge to help me assist people with health needs. I am not in the industry to make money. I am here to help people in need and make a difference in their lives. I had worked with different organizations for so many years and now, I opened my own company to help people remaining in their houses for as long as they need and want. I had a lot of different nursing skills that help me give quality care to my clients.”
